The Institute can initiate the proceedings from your side, by getting following documents in our possession:
- An authority letter from the candidate in the name of Institute, that the Institute is entitled to collect the eligibility certificate.
- A Bank draft of US $ 500/- in favour of Registrar, Baba Farid University of Health Sciences, Faridkot.
- In addition to this a sum of Rs. 1000/- is payable in cash towards the cost of the form of application of the University.
As per university requirement the original documents along with its photocopies duly attested by a competent authority (Notary of the country/province or any other authority in the country eligible to attest the documents)) are required at the time of seeking of Eligibility Certificate. Only on getting this certificate the candidate will be able to apply for seeking admission. Documents required are given below:-
- Application form obtaining equivalency/eligibility certificate duly signed by the candidate.(Copy of form enclosed)
- 10+1/equivalent examination detailed marks certificate(s). The certificate(s) must show the marks of Physics, Chemistry, Biology and English.
- 10+2/equivalent examination detailed marks certificate(s). The certificate(s) must show the marks of Physics, Chemistry, Biology and English.
- Explanation Sheet of grades (Transcript in a sealed cover).
- Pass Certificate/Diploma of 10+2 or equivalent examination.
- Detail of Grades of Qualifying examination.
- Date of Birth Certificate.
- Character/Recommendation Certificate from the Head of the Institution last attended.
- Proof of NRI status of candidate/parents.
- Two copies of Passport size photographs
- Copy of Provisional student visa or OCI or PIO as the case may be.
These documents should be original along with photocopy of each document duly attested by a Notary of the Country/Province).
- The copies duly attested by Notary will be retained by the University.
- Transcript in original will also be retained by the University.
- The rest of the original certificates will be returned to the institute after processing the Eligibility Certificate.
